Veterinary Approval & Health Information
I understand that veterinary approval is not required for every horse or every bodywork session. However, Equi-Librium Bodywork reserves the right to require veterinary approval or consultation before providing or continuing services when required by applicable state law or when, in the practitioner’s professional judgment, the horse’s health history, current condition, recent treatment, injury, illness, lameness, or other circumstances warrant veterinary involvement.
I understand that if veterinary approval is requested, bodywork services may be postponed or declined until appropriate approval or guidance has been received.
I certify that, to the best of my knowledge, the information I have provided regarding my horse’s health, medical history, medications, injuries, lameness, veterinary care, behavioral history, and current condition is complete and accurate.
I agree to notify Equi-Librium Bodywork of any significant change in my horse’s condition prior to a scheduled session, including but not limited to: illness, injury, lameness, heat, swelling, fever, wounds, surgery, injections, medication changes, or other veterinary treatment.
I understand that failure to disclose relevant information may affect the safety or appropriateness of bodywork.
Practitioner Discretion
I understand and agree that Equi-Librium Bodywork may, at its professional discretion, modify, limit, postpone, or discontinue a session if the horse’s physical condition, behavior, environment, available information, or another circumstance raises a concern regarding the safety or appropriateness of continuing.
I understand that Equi-Librium Bodywork may recommend that I contact or follow up with a veterinarian, farrier, saddle fitter, trainer, or other qualified professional when appropriate. Such a recommendation does not constitute a diagnosis.
Horse Response & Results
I understand that every horse responds differently to bodywork and that no specific result or outcome is promised or guaranteed.
I understand that a horse may experience temporary changes following a session, including changes in movement, energy level, sensitivity, behavior, muscle tone, or general demeanor. I agree to contact my veterinarian regarding any concerning, significant, persistent, or worsening symptoms.
I understand that recommendations made following a session are intended as general supportive guidance within the practitioner’s scope of practice and do not replace veterinary instructions or treatment plans.
Assumption of Equine-Related Risk
I understand that horses are large, powerful, and inherently unpredictable animals. Even a horse with no known history of dangerous behavior may react suddenly or unexpectedly.
I acknowledge that working around horses involves inherent risks, including but not limited to kicking, biting, striking, stepping on a person, pushing, pulling, bolting, rearing, falling, sudden movement, collision with animals or objects, equipment-related incidents, and unpredictable reactions to touch, pressure, sounds, people, animals, environmental conditions, or other stimuli.
I voluntarily assume the inherent risks associated with my presence and participation during equine bodywork activities and with handling or being in proximity to my horse before, during, and after a session.
Owner/Handler Responsibilities
I agree to disclose any known history of dangerous, aggressive, unpredictable, or difficult behavior that could reasonably affect the safety of the practitioner, handler, horse, other animals, or other persons.
Unless otherwise arranged, I agree that I or another competent and authorized handler will be available to safely handle the horse as needed.
I understand that I am responsible for providing, or arranging access to, a reasonably safe and appropriate location for the session.
I understand that Equi-Librium Bodywork may immediately discontinue or decline services if the horse, handler, environment, equipment, or surrounding circumstances present an unreasonable safety concern.
Release & Waiver of Liability
To the fullest extent permitted by applicable law, I knowingly and voluntarily assume the inherent risks associated with equine activities and agree to release and hold harmless Equi-Librium Bodywork, its owner, practitioner(s) and representatives, from claims for personal injury, death, or property damage arising from the inherent risks of equine activities or my own acts in connection with services provided by Equi-Librium Bodywork.
I understand that this agreement does not eliminate responsibilities or liabilities that applicable law does not permit the parties to waive.
Responsibility for the Horse
I represent that I am the legal owner of the horse identified in this intake form OR that I have been authorized by the legal owner to request and consent to bodywork services on the horse’s behalf.
I accept responsibility for decisions regarding my horse’s care and for obtaining veterinary evaluation or treatment when appropriate.
I understand that Equi-Librium Bodywork is not responsible for the progression of a pre-existing, underlying, known, or unknown medical condition solely because bodywork services were provided.
Emergency Situations
If an urgent health or safety concern becomes apparent during a session, I understand that the session may be stopped immediately.
I authorize Equi-Librium Bodywork to take reasonable steps necessary to protect the immediate safety of the practitioner, horse, handler, and surrounding persons. I understand that I remain responsible for contacting and obtaining veterinary care for my horse.
Continuing Consent
Unless revoked by me in writing, I understand that this agreement applies to the initial session and subsequent services provided by Equi-Librium Bodywork for this horse.
I understand that it is my responsibility to provide updated information when my horse’s health, veterinary care, medications, condition, ownership, or other relevant circumstances change.
Cancellation Policy
I understand that cancellations or rescheduling requests should be made at least 24 hours piror to the scheduled appointment. Appointments cancelled with less than 24 hours' notice, or missed without notice, may be subject to the full scheduled session fee.
I understand Equi-Librium Bodywrok reserves the right to waive the cancellation fee when appropriate due to emergencies, unsafe weather or travel conditions or other extenuating circumstances.

I have carefully read and understand this Consent, Assumption of Risk & Release of Liability.
I have had the opportunity to ask questions before signing.
The information I have provided regarding my horse is accurate to the best of my knowledge.
I understand the nature and limitations of equine bodywork.
I understand that bodywork does not replace veterinary diagnosis or treatment.
I understand and voluntarily assume the inherent risks associated with horses and equine activities.
I consent to Equi-Librium Bodywork providing bodywork services to the horse identified in this intake form.
I understand that I may withdraw consent for bodywork at any time.
I understand the cancellation policy, including that cancellations made with lass than 24 hours' notice or appointments missed without notice may be subject to the full session fee.
